Could you please study the details in the attached files and send us quotation summary of at least 4 agencies upon PSI’s requirement, paying attention to the followings:

1. Use attached PSI quotation form No. PSI-SVC-22-05 Personal health and Accident Insurance 2022-2023 [1]
2. Properly sign and stamp your quotation
3. Indicate outside the envelop with: Quotation “2022-2023 Personal accident & Health insurance – No. SVC-22-05”
